Liberty Lake Community Yard Sales today boasts more than 260 homes

"I called out of the blue," she told Maclean's in a recent interview. "I wanted to talk to her if she was open to chatting. I talked to a couple of her sisters, and upon getting information from them, they said, 'Would you like to speak to her now?' " Morissette remembers weighing the decision in her mind. "In that moment taking into account how I felt when a lot of people wanted to speak to me I just said, 'No thanks, I'm sure she's really tired.' Then the next morning I woke up and she had died."

By now, anyone who has heard Morissette's recent hit single Thank U ("thank you India, thank you providence") knows that she, like the Beatles, made the pilgrimage to the East and came back transformed. The sexy anger of Jagged Little Pill has given way to healing and reconciliation. Morissette turns free form journal entries into unrhyming lyrics. The first showed her as a rock chick in the desert vamping through You Oughta Know in black leather pants; by the fourth video, for Head over Feet, she posed without makeup, giggling at the camera, in a dramatically unflattering close up. Then for Thank U, she walked the streets of downtown Los Angeles in a granola/Godiva pose of sexless nudity, her breasts coyly veiled by the hippie hair and her crotch whited out by a video blur. "I was in my shower when I thought of the idea," she says, adding she would happily bare all if it were not for censors. "When I'm naked, I feel so free and liberated and unself conscious and close to God. So I thought it would be appropriate to be naked in my video."

Record company executives were not so thrilled. "They would have loved something safe," says Morissette's Los Angeles based manager, Scott Welch. "But she felt, 'I need to let girls see that you can just be who you are.' She's fearless. Here's a star who's not afraid to be shown without perfect hair or makeup. And she's not afraid to fail in public."

The photo shoot is set up in a basement room of Toronto's new sports arena, the Air Canada Centre. There is a toxic odour of fresh carpet in the air. Alanis, fresh from a news conference announcing the Canadian tour, walks in wearing a long knit coat. Underneath, a delicate brown blouse with sheer sleeves, button fly blue jeans, scuffed black combat boots. Her hair is a loose mane. Posing for a photographer, Morissette meets his eye with the plain, open gaze of an innocent girl. But she is a wary subject, with the self possession of a woman beyond her years. When asked to lower her head and look up at the camera, she hesitates.

And me not knowing when to say no and when to say yes and perhaps erring on the side of saying no." On the other hand, she adds, "fame was an amazing way to see who I was dealing with pretty much right away. I can immediately tell a lot about someone's character by the way they feel about fame."

Then there are the perks. Getting to hang out with other famous people. Going to Cuba with Leonardo DiCaprio. "It was a cultural exchange with 20 people put together by an investment banking company in New York," Morissette explains. "We went to different hospitals and art galleries and restaurants and dance clubs and really just absorbed it. Basically, the Cuban culture wants America to see them as more than just an embargo situation, and they felt inviting different artists down might result in us kind of spreading the word." The trip was an all inclusive package. "One minute I'd be in an AIDS hospital speaking to a patient and connecting with them, and later that night I would be dancing my head off at an outdoor salsa club. It was everything. It was beautiful and inspiring."

And romantic. After meeting Dash Mihok in Cuba (he's the red headed Pte. Doll in Thin Red Line and a regular on TV's Felicity), a year later Morissette says they are still in love. So how many times has she been in love? "We have to define what we feel 'in love' means," she replies. "In love, I think maybe once. I have felt infatuation and heart palpitations and obsession countless times. But I've only been in one really healthy relationship. And that's this one."

Philadelphia, the eve of Valentine's Day. Some 15,000 fans packed into a hockey arena erupt into wild cheers as Alanis steps onstage. She is clad in black, with a spangled skirt wrapped around loose pants and a sleeveless top. Picking a microphone up off the floor, she launches into Baba from the new album, a rock dirge about Western tourists seeking instant salvation at the feet of "makeshift gurus." The theme of Third World exotica extends to the simple stage set, which is flanked by batik banners, and backed by pandora 2016 ornate Moroccan fretwork.

But for a rock spectacle, it is a no frills production. And Alanis wins over the audience without the usual rock 'n' roll stagecraft. She does not dance so much as stride back and forth to the beat. Happily lost in her own world, she lets people watch the spirit move her. Nor does she interact much with her musicians a drummer, two guitarists, a bassist and a keyboard player. Her voice carries the show. It is a powerful, acrobatic voice, one that is comfortable up in the jet stream somersaulting in and out of falsetto. As she sings, she does something weird with her hands. Holding the microphone with her right hand, the left hangs limp and twitches a spastic code, fingers selecting hieroglyphic cues out of the air.

Between songs, stopping each time to sip from a water bottle that she keeps by the drum kit, she is coolly dispassionate. At one point, she glances up at the arena's digital clock and announces: "It's 10:14 and all is well." Talk about being in the moment.

The audience responds best to the hits. And with the opening bars of Thank U, it is truly bizarre to hear cheers of recognition greet lines like "how 'bout getting off of these antibiotics" and "how 'bout them transparent dangling carrots." One Hand in My Pocket starts out unplugged, to the sound of hand drums and harmonica, while a video shows Alanis jogging into the sunset. Then, in You Learn, she lets loose. With a snowboarding video playing behind her, Alanis jumps and spins in circles, faster and faster, like a kid trying to make herself dizzy, until her braid is pinwheeling around her head. A whirling dervish. As the song ends, she folds her arms in prayer.

None of this seems choreographed. Or, at least, the illusion of navet is convincing. And none of the rage and pain underlying Morissette's lyrics comes across in performance. From the Sgt. Pepper thrum of the band to the Indian decor, the spirit is born again peace and love. It is white music while the majority of Philadelphia's population is African American, there are few black faces in the audience and it's not terribly sexy. Whenever things threaten to get too ethereal, however, Alanis picks up the harmonica. She is no virtuoso, but as she bends over and blows the living daylights out of it, she drives the crowd to a frenzy with her sheer exuberance.

Later, when asked about her fondness for the harmonica, Morissette says: "It's a sweet little instrument, yeah. It's coming from the mouth. I love wind instruments." Alanis heard a lot of harmonica as a child: her parents were heavily into Bob Dylan.

North Korea threatens nuclear strike on US as UN votes for tough sanctions

The move sparked a furious Pyongyang to threaten a nuclear strike against the United States.

The vote by the UN's most powerful body on a resolution drafted by North Korea's closest ally, China, and the United States sends a powerful message that the international community condemns the ballistic missile and nuclear tests and repeated violation of Security Council resolutions.

Immediately before the vote, an unidentified spokesman for Pyongyang's Foreign Ministry said the North will exercise its right for "a preemptive nuclear attack to destroy the strongholds of the aggressors'' because Washington is pushing to start a nuclear war against the North.

It appeared to be the most specific open threat of a nuclear strike by any country against another.

Although North Korea boasts of nuclear bombs and pre emptive strikes, it is not thought to have mastered the ability to produce a warhead small enough to put on a missile capable of reaching the US It is believed to have enough nuclear fuel, however, for several crude nuclear devices.

The new sanctions are aimed at reining in North Korea's nuclear and missile programs by making it more difficult for Pyongyang to finance and obtain material for these programs, tracking illegal diplomatic activity and intensifying inspections of cargo to and from the country.

The international community has condemned the regime's nuclear and missile efforts as threats to regional security and a drain on the resources that could go to North Korea's largely destitute people.

The UN resolution identifies three individuals, one corporation and one organisation that will be added to the UN sanctions list. The targets include top officials at a company that is the country's primary arms dealer and main exporter of ballistic missile related equipment, and a national organisation responsible for research and development of missiles and probably nuclear weapons.

The success of a new round of sanctions could depend on enforcement by China, where most of the companies and banks that North Korea is believed to work with are based.

3 men charged with murder in deadly Passaic nightclub shootings

A 24 year old man, an unskilled worker in a small shop, was admitted with a two day history of shortness of breath and altered sensorium. He had been admitted to a government hospital twice previously. The first time, 4 years earlier, he had edema and high blood pressure. The 24 hour protein excretion was 3.2 g; creatinine clearance, 26 mL/min; serum creatinine, 3.8 mg/dL; and blood urea, 100 mg/dL; ultrasound examination disclosed bilaterally small kidneys. A renal biopsy was not performed. He was given amlodipine, 10 mg twice daily; prazosin, 5 mg twice daily; and calcium carbonate, 500 mg three times daily with meals. He was advised to monitor his blood pressure and be monitored regularly. He did not comply with the advice.

The second admission occurred six weeks ago, when he presented with advanced azotemia. He underwent two hemodialysis sessions through a double lumen femoral catheter and was discharged and advised to continue hemodialysis as an outpatient and look for a kidney donor in the family. He defaulted until the current admission.

On examination he was malnourished with obtunded sensorium. The pulse was 116 beats/min and regular; blood pressure, 90/70 mm Hg; and respiratory rate, 30 breaths/min. He was severely anemic with uremic fetor, anasarca, and a raised jugular venous pressure. Cardiomegaly was present, as was pulsus paradoxus of 20 mm Hg, and cardiac sounds were muffled. Chest examination disclosed basal crepitations. Examination of the abdomen revealed hepatomegaly and ascites. Central nervous system examination revealed that the patient was in grade III coma with no localizing neurologic signs. He was clinically diagnosed as having advanced renal failure with pericardial effusion. Investigations revealed 3+ urinary albumin, no sugar, and 4 to 5 granular and broad casts/high power field; hemoglobin, 4.8 g/d; hematocrit, 14%; white blood cell count, 8700/mm3; and platelet count, 260,000/mm3. A peripheral smear revealed marked anisocytosis and hypochromia. Blood sugar was 80 mg/dL; blood urea, 380 mg/dL; serum creatinine, 22 mg/dL; uric acid, 14 mg/dL; sodium, 126 mmol/L; potassium, 5.4 mmol/L; chloride, 94 mmol/L; total protein, 5.0 g/dL; and serum albumin, 2.2 g/dL. The serum calcium was 8.2 mg/dL; phosphorus, 10 mg/dL; and alkaline phosphatase, 230 IU/L. Arterial blood revealed pH 7.2; PCO2, 28; HCO3 , 12 mEq/L. A chest radiograph disclosed severe cardiomegaly with congestion in the lung parenchyma. Echocardiography revealed a large anterior and posterior pericardial effusion. An emergency pericardiocentesis drained 600 mL of hemorrhagic fluid. He also received 3 units of packed cells and hemodialysis through a femoral catheter. He developed fever and breathlessness on day 4 of his admission, had to be ventilated for acute respiratory failure secondary to bilateral staphylococcal pneumonia, and died on day 7.

Patient 2. A 68 year old businessman with a 20 year history of diabetes mellitus initially controlled on glibenclamide and metformin was now controlled on injectable human insulin, 40 U/day. He also had hypertension of 10 years duration that was controlled currently with clonidine, 0.6 mg/day, and amlodipine, 10 mg/day. He had had pedal edema one year previously. The 24 hour urinary protein was 2.4 g; creatinine clearance was 24 mL/min. His vision had been affected and an ophthalmologist had treated both eyes with laser. He was admitted now with loss of appetite, occasional vomiting, and weakness. He denied chest pain, claudication, or urinary complaints.

Physical examination revealed a pleasant elderly gentleman. His blood pressure was 150/80 mm Hg; pulse, pandora 2016 80 beats/min and regular. All peripheral pulses were palpable, he had no carotid or renal bruit, respiratory rate was 18 breaths/min, and pedal and leg edema were present. Temperature was 98 weight, 84 kg; height, 5'8". He was anemic, and the jugular venous pressure was not raised. He had cardiomegaly and sensory peripheral neuropathy.

Laboratory investigation revealed a 24 hour urinary protein of 1.8 g; creatinine clearance, 7 mL/min; urine culture, sterile; hemoglobin, 9.6 g/dL; white blood cell count, 9400 mm3, unremarkable differential counts, erythrocyte sedimentation rate, 34 mm at first hour; and a peripheral smear revealed normocytic normochromic anemia. Serum iron was 98 ng/mL; total iron binding capacity, 420 ng/mL; and ferritin, 148 ng/mL. Blood urea was 200 mg/dL; serum creatinine, 14.4 mg/dL; electrolytes were relatively normal; serum calcium, 8.8 mg/dL; serum phosphorus, 6.8 mg/dL; alkaline phosphatase, 124 IU/L; and pandora jewelry outlet intact parathyroid hormone, 54 pg/mL. Liver function tests were normal. A chest radiograph revealed moderate cardiomegaly. Doppler ultrasonography of the carotid artery and abdominal aorta did not reveal any plaques. His serologic markers, including HBsAg, HIV I and II, VDRL, anti HCV, and IgM CMV were negative; the IgG CMV was positive. His blood group was B+. He was diagnosed as having end stage renal disease, and options of renal replacement therapy were discussed with the patient and his family. His 60 year old wife, whose blood was O+, volunteered to be the donor. Two brothers and one sister were diabetic and therefore not considered. Two sons, 34 years old and 30 years old, and a daughter, 32 years old, were keen on donating, but the patient refused to accept their donating a kidney to him. His wife was fully evaluated and accepted as a donor. He received 2 injections of basiliximab and triple immunosuppression consisting of cyclosporine, mycophenolate, and steroids. He is doing well without any complications one year after transplantation and has normal renal function.

VIJAY KHER (Senior Consultant, Department of Nephrology, Indraprastha Apollo Hospitals, New Delhi, India): These two cases illustrate the great contrast in and diversity of care available to patients in the developing world who have end stage renal disease (ESRD). The first case highlights the tragedy of most patients in the developing world who, without state funding, have no or only meager resources for paying the recurring cost of ESRD care. The second case illustrates that patients with means can avail themselves of facilities as good as those anywhere in the developed world. I will discuss the issues pandora charms on sale of the burden of ESRD, availability of care for these patients, estimated proportions of patients receiving different modalities of care, economic barriers, and costs and ethical issues involved in the care of ESRD in developing countries. An earlier Nephrology Forum described the care of the ESRD patient in China1. In this Forum, I will use India as a prime example for addressing these issues.

Chronic renal failure is a devastating medical, social, and economic problem for patients and their families. The availability and quality of dialysis programs largely depend on the prevailing economic conditions, the political social structure, overall health care facilities, and the health care funding strategies of various countries. Large disparities separate the socio economic structures of various countries, especially the developed and the developing countries Tables 1 and 22. In the developed world, health care is generally available, whereas the vast population of people living in developing countries do not have access to even basic amenities like sanitation and safe drinking water. For ESRD patients in economically advanced countries, the focus now is on improving quality of life and increasing long term survival. In marked contrast, the developing countries are grappling with short term patient survival and the enormous costs of therapy that limit continuation of treatment in the majority of patients with ESRD. Most of the developing world has a two tier health care delivery system. In the government run non profit hospitals, patients do not have to pay for medical advice, basic examinations, or treatment, but they must pay for disposables (gowns, gloves, syringes, for example) and drugs. However, in the large number of private hospitals, patients do have to pay for all services. Unlike the developed world, most developing countries lack renal registries. Therefore, the exact incidence and prevalence of chronic renal failure in the population, its burden on the health care system, and the outcome of these patients are not known. Most of the available data, including those from the most populous countries like China and India, reflect the collected experience from a few individual physicians and few tertiary centers and do not reflect the situation in its entirety3. The reported annual incidence of patients with ESRD varies widely, from as low as 4 per million population (pmp) in Bolivia to as pandora jewelry 70% off clearance high as 254 pmp in Puerto Rico4,5. HemodialysisIn India, with a population base of one billion and an estimated incidence of ESRD of 100 pmp, approximately 100,000 patients develop ESRD each year. Of these, 90% never see a nephrologist. Of the 10,000 patients who do consult a nephrologist, renal replacement therapy is initiated in 90%; the other 10% are unable to afford any form of RRT9. India has no governmental reimbursement for dialysis or transplantation, and only a small percentage of patients with ESRD have employer sponsors or health insurance that pays for RRT. Of the patients referred to a nephrologist, approximately 30% to 40% are critically ill and require immediate dialysis because of symptoms that include pulmonary edema, pericardial effusion, severe metabolic acidosis, and encephalopathy10.

The majority of the 9000 patients who receive RRT are begun on hemodialysis. Only a small proportion (11,12. Approximately 9% to 13% of patients die within one year while being treated. Between 17% and 23% of patients undergo renal transplantation after being on dialysis for 2 to 3 months for pre transplant stabilization. Although about 4% of patients remain on maintenance hemodialysis Figure 111,12, very few stay on maintenance hemodialysis longer than 24 months. Among the reasons for this difference are the delay in detecting renal disease and the failure to institute controlling and preventive measures in patients with progressive renal failure, both of which result in faster deterioration of renal function and progression to ESRD. About 61% of patients with chronic renal failure present with ESRD to specialists12. Late referrals lead to a faster progression of co morbid conditions, increase the cost of therapy, and worsen overall patient survival.

The relative prevalence of various primary renal diseases among patients with ESRD is highly variable in different countries. Chronic glomerulonephritis, diabetic nephropathy, chronic interstitial nephritis, and hypertensive nephrosclerosis are the leading causes of ESRD in India Table 312,13,14,15. In patients between 40 and 60 years old, diabetic nephropathy is the most common cause of ESRD12. Only 10% to 15% of patients are biopsied, as the vast majority of patients with chronic renal failure have bilaterally contracted kidneys by the time they present to a nephrologist. Treatment facilities for end stage renal disease are not available uniformly to all sections of society in most of the developing world. Women, children, and the elderly bear the brunt of such discrimination and are often not offered dialysis. The number of centers providing regular dialysis services varies from 1.5 pmp in the UK to 20.3 pmp in Japan17. The number of dialysis units in developing countries is not known, although published figures indicate that the fewest are in the large populated countries: pandora uk sale Brazil, 3.3 pmp5; Algeria, 0.8 pmp18; China, 0.2 pmp1; and India, 0.2 pmp3. Patients assigned to hemodialysis are usually dialyzed only twice weekly, each session lasting 3 to 4 hours. Patients usually are dialyzed on cellulosic membranes with a surface area of 1.0 to 1.3 m2 at blood flow rates of 200 to 300 mL/min using 16 G fistula needles. Acetate dialysate is used in more than 70% of patients for economic and technical reasons. The dialyzer and tubing are reused together four to six times, with manual reprocessing using formaldehyde for disinfection.

In India, the prescription for hemodialysis typically is empiric. Data on adequacy of such dialysis schedules are not available. Rao et al reported a short study indicating that KT/V was less than 1 in about 50% of the hemodialysis sessions studied. Higher KT/V and urea reduction ratios (URRs) were associated with smaller body size, use of arteriovenous fistula rather than a venous access, and higher dialyzer to body surface area ratio19. The assumption that a satisfactory KT/V and time averaged concentration (TAC) of urea can be achieved in Indian ESRD patients with twice weekly dialysis needs to be tested in rigorous clinical trials. In my view, most patients continue to suffer from uremic symptoms on such dialysis schedules, and long time survivors are few and far between. There has been a significant improvement in countries where governments have earmarked separate funds for dialysis as in eastern European countries8.

In most Indian centers, water treatment, if any, consists only of de ionization. Often water analyses are not performed before one decides on the water treatment required. In only a few centers is water treatment appropriate, with activated charcoal and reverse osmosis. Water testing facilities do not exist everywhere. Water quality is tested infrequently or never.

In underdeveloped countries, a combination of poor living conditions, inadequate dialysis, malnutrition, hypoalbuminemia, and frequent blood transfusion makes dialysis patients prone to a variety of bacterial, viral, and fungal infections. Infection and cardiac diseases remain the predominant causes of death in Indian dialysis patients11,12. The incidence of tuberculosis in dialysis patients has been reported to vary between 4% and 15%20. Viral hepatitis is among the most common viral infections encountered in dialysis patients. The incidence and prevalence of hepatitis B viral infection varies from country to country. But hepatitis B vaccination among dialysis patients, despite low seroconversion rates, has reduced the prevalence rates, from 32% to 4.7% even among patients in endemic areas21. In recent years, hepatitis C virus (HCV) has emerged as the primary cause of viral hepatitis in patients undergoing regular dialysis therapy22. The prevalence varies between 16% and 82%23. Nosocomial transmission has been demonstrated in dialysis units.
